This is a full Drupal 7 package that includes custom example modules for stylizing and otherwise enhancing forms, created to augment a presentation called "How to Build Beautiful Forms in Drupal 7". View the /assets folder for a database dump, or you can simply go through the Drupal installer. All custom modules are in /sites/all/modules/custom/beautifulforms.
